Possibility of altering templates through a data input page?

Hi,

I was just wondering if anyone could help...

I'm using Word 2003 and I wondered if there was an option to create a data
input page in Word so that I can enter all the information I need to alter a
template on the first page which automatically changes the content of the
document in the specific places, such as name and address and whether I can
also change about 10% of the template text easily to meet the needs of an
individual agreement?
