Hello,
I am looking for a form that I can send around internally where
recipients are asked Yes/No and ranking 1-5 and comments fields to a
series of questions. I have downloaded and tried
http://www.outlookcode.com/d/forms/survey.htm and got our IT person to
look at it but after several hours gave up. The form never appeared
correctly for the recipient and didn't delivery back any results to the
sender.
Does anyone know where else I can get survey forms? what I am looking
for is just 3/4 types of data fields. thanks.
