Primary Key not Duplicating

One-to-many releated table. The record is put in the one, and then records
are added to the many. When we go back to check the record, the one made it
in, but for some reason the primay key is not associated with the many. This
happens on an inconsistant basis. The values for the many are in the table
without a key assigned (basically lost records).

Does anyone have any ideas what could be going on here?

Have you established the relationship and set to cascade update?
