Print error - numbers instead of text

I have one form that when you look at the print preview it appears to look
good, but when you actually print it there are a bunch of numbers over where
the text and text boxes should be. It looks awful and can't be read at all.
I don't understand why only this form is giving me this issue. I checked out
blogs and goggled it, but did not find my exact issue. Please help!!!

Hello,

Do you have any conditional formatting for text fields? Where you are
getting the numbers is those are related to dropdown values?

Do you have table and section formatting the fields?

Hi Shiva,

No, all I have are questions (text) and text fields for the answers, check
boxes and drop down lists. No conditional formatting. No table or section
formatting either. I don't know where the numbers are coming from.

It is also perplexing why the Print Preview appears fine, but it is the
actual hard copy that shows gibberish. Your help is greatly appreciated.

Can you post the pic of the printed form, It might help us to understand
better.
Anything changed in the Printer settings?

Hi Kalyan,

How would I post a picture/document? I scanned the hard copy and have it
..pdf format. I tried copying and pasting, but that does not appear to work
on the post.
