printing 3 x 5" index cards with Word for Mac X

I have the paper size set to 3x5". The print preview is perfect. Yet
when I print, the printer spits out a blank card.
When I put in a letter size piece of paper, it prints the text from the
card at the very bottom left of the sheet.
What am I doing wrong?
