printing "Contacts" address book - memo style

I'm trying to print all my Contacts in order to have a
hard copy and use as an address book.

When I print (using Memo style) instead of printing the
persons's name at the top of the page (before the line)
each page prints out with "Unknown" at the top. What do
I have to do to get the person's name at the top of the
page.

The name that prints at the top of the page is the name associated with the
default e-mail account for that mail profile. If there are no e-mail
accounts configured, "Unknown" will print.
