Printing custom forms in Outlook

I have created a custom form in Outlook 2000 and used the ActiveX Print
add-in in order to print the "received" form. However, despite using the
standard "From" field in the form and correctly binding it, the value (eg.
the sender) is not appearing in the printed version, although it is showing
when I look at the screen version.

Can anybody help please (I'm starting to get more than a little bit
frustrated now...)

I have done this on a form. I added the From field from the Field Chooser
list. Then, I kept the From button, but replaced the recipient control field
with a regular textbox control and bound that control to the From field under
Frequently Used Fields.
