printing forms in outlook 2007

Hi there,
Just installed Outlook 2007 (previously had 2003) and I have the need to
print a customised form.
In my form I have a series of text boxes and YES - NO select boxes under
different headings. Some of these are Accommodation, Airfares, Departure
Date, Departure Time, Car Hire etc etc.
The problem I have with the printing is that;
It prints the headings in Alphabetical order and this causes them to be "out
of whack" with my original form order. (I have Airfares listed first on my
form then departure etc but when printed it lists Accommodation first,
Airfares, Car Hire, Departure etc etc)

1. - Can the order of printing be fixed / Changed?
2. - Can I print it as it apperas exactly on the screen? (I know i could use
ALT Print Screen and paste it into a document but the form requires scrolling
to view it all and thus won't fit in a single screen dump!)
