Printing on index cards

I occasionally print on 3x5 or 4x6 index cards. I'm having trouble doing this with Word 2000. (1) no index card paper selections, as there had been in Word 97, (2) defining a custom size does not seem to retain, keep reverting to the next closest size in the available selections, (3) when printing, it appears to skew off the card to the left. Maybe I'm missing a configuration setting somewhere, everything I see seems set appropriately. Any suggestions? Thanks
