Problem With a Form

I took your advise and redesigned the form on my web site to eliminate
the radio buttons and use the check boxes instead. However I still have
the same problem where the confirmation and email sent to me are missing
several of the fields. When I look at the page source all the fields
seem to be listed.
PLease do you have any more ideas, our members are starting to use this
form and I really need to get it working properly.

Recommendations:

do the bscc member checkbox then the ahcso checkbox and then one checkbox
for yes I will serve

previous exp. and no prev. exp radio buttons should be a radio group, you
can only be one OR the other

testing:

I tested the page and found the earlier problem of mismatched groupings is
cleared up, and found that on the front and rear gate slections they miss a
couple of time selections. Look over the control properties of each of those
checkboxes and double-check their settings you may have something whacked
there. Also make sure none of the controls are grouped or overlapping.

If you cover all possible things and have no luck with it you can send me

Thanks for the fast reply. I will change the other buttons tonight. Would
overlapping text boxes with controls cause the missing time selections.
