Problem with approuving a form

I've ceate a form that need to be approuved or refused. If I send m
the form and approuve it, it doesn't do the custom Action. I
re-send me the form with the subject changed. If I re-approuve thi
form : it do the custom action. Do you have any idea of what i
missing in my form

What do you mean by re-approve the form? What are you expecting the Approve
custom action to do that it isn't already doing? WHere is the form published?
