Problem with check box in a custom form

I have an outlook form which has a check box field. If I
check the field once, I am unable to un-check it. What
am I doing wrong?

Second, how do I assign an input mask to a phone field in
outlook form. I would like it to display as (214)555-
1212.

Did you bind the check box to a Yes/No field?

Text boxes on Outlook forms don't support input masks. If you want Outlook's automatic phone number formatting to kick in, either don't customize the first page or use code to copy the number to a new item and have that item do the formatting.
