Problem with secure forms

I'm having a problem getting a secure form to work. When the user submits the information, instead of sending it to the proper place and redirecting the user to the confirmation page, it sort of "refreshes" the form, but with all the graphics Xed out, doesn't submit the info, and the address is something like http://www.mypage.com/_vti_bin/shtml.exe/page.htm. This only happens with forms that are on our secure server, and it seems to work sometimes, and then not work other times. Why does this happen, and how can I fix it?
