Programmatically add an item to a SharePoint list fails

Hello all,

I have followed this tutorial,

with slight modifications to fit my needs but it seems like when i hit
submit, nothing shows up in my sharepoint list. no errors or anything.

my code seems alright, my GUID is correct, im not sure what to check from
here. any ideas?
