Project Corrupted from adding a new task

A user of mine tried to add a new task to a project. Project came back and
said that a duplicate index would be created because of this addition. The
user cancelled adding the task and closed the project without saving. After
that the entire project plan disappeared. I have since recovered the project
from a backup but am wondering if this was a fluke error or if it was
actually caused by something that I should fix?

If, it's something fixable, then it's the behavior that caused the dupe
index entry. The most likely and common cause is cutting and pasting entire
task rows within or between plans that are published to Project Server. Stop
the causative behavior and the symptoms will end.
