Project Name Dialog Box

I have created a protected Word document with user fields in MS Word
2003. I then saved the document as a template. Now when I open the
template file, a text box appears titled "Microsoft Office Word" and
has a message that reads "Project Name". Any text entered in the text
box appears in the new document based on the template, but then I can
not modify the text.

My question is, what is causing this text box to appear and how do I
prevent it from appearing.
