Project Server 2003 - document links to issues disappear

I am able to link documents to issues; however, these links disappear over
time. I haven't been able to associate this disappearance with any sort of
event. In fact, it appears that a number of links disappeared over the
holidays while no one was even using project server. Any Ideas?
