Project Server 2007 - 2 timesheet review jobs submitted to Job Que

When a PM reviews/approve timesheet, it generates one Timesheet Review job
in the job queue. For some users, we are seeing 2. One runs successfully, the
second end up "Failed And Blocking Correlation" state and needs to be
cancelled. Any idea on cause, or what/where to look for? We use fixed
approval routing so users cannot change timesheet approval routing - it goes
to timesheet approver.
