I'm looking for a project server user satisfaction survay questioner.
If someone can advice if he/she had done it and could provide a copy of the
questioner, Or advice where can I get one.

Ein,

You should be able to use the survey functionality in SharePoint to do this
survey. We set up a subweb with anonymous access turned on for the surveys.

As for the questions, it really depends on what you want to measure. Since
the possibilities are so broad, can you provide some guidance?
