Proposal Workflow not working

I've configured workflow as suggested on technet
(http://technet.microsoft.com/en-us/library/cc197595.aspx). However, when I
create or change a proposal there is no email notification sent nor any
documents created in Shared Documents eventhough I have supplied the name(s)
of people who should receive them.

What else do I need to do?

PS. Yes we have MOSS installed.

I had already raised this issue before but did not get a resolution,

The only place I have seen this working is in the EPM VM environment
distributed by Microsoft. I have built a number of environments where
normal MOSS worklows work fine but not proposals and without any
apparent error diagnostics. I note also that responses to Brian Smiths
blog on this topic flagged a similar problem. I found this article
which suggests there are issues but was not generally helpful
