Public Folders

Are public folders only available in a domain if you have an Exchange email
account?

Kinda sorta...

The public folders can be made visible to the internet via anonymous access.
However, if you wish to use the public folders, and you have an
exchange server, why do you not have an account?

With an account you still need to be given security permissions to have
public folder access.

Can you explain in a bit more detail what your ultimate goal is?

Sure,
I have a customer who is very "frugal" and they have an Exchange server that
only about half of their employees have accounts on. They are wanting to use
a public folder to post messages that the entire organization can view,
without creating Exchange accounts for all users. If they did add all users,
they could just use email to dispense messages, but they are wanting to do it
cheap.
