Publish Access 97 form to web

I ran through the Web publishing wizard, created the ODBC
driver and made sure the link to the ASP pages on the
server is correct.

But when I open the page, I get a little red "X" in the
top left corner.

I need to enable users to enter information into the
database through a form on the web.

Any ideas?
