Pull-down menus within a Word document

I've downloaded a 64KB application form, a Word document that includes
pull-down menus, the only way to complete some items on the form. But
the menu choices do not show up when I open the document in Office:mac
2004 (OS 10.3.9). Suggestions? Thanks.

Are the pull-down menus ActiveX controls in the document itself? If so,
they won't work in MacOffice.
