Query by Form in a Data Access Page

I have not worked with data access pages before. How can I create a DAP that
has text boxes to allow users to input query parameters. Basically I need to
do a query by form in a DAP. I'd like to be able to do it in a single page
(form/subform) like what I have in the Access file itself.
