Query by Form

On a cd that came with an Access 2000 textbook there was a simple utility
that took an existing form and made a version that could be used as a QBF
(ie you could query on all fields that were in the original form). The book
was probably Osbornes Complete Reference to Access 2000.

Unfortunately I've lost the cd - and am also now on Access XP.

Does anyone know if there is a version of this utility for XP (or that gives
me the same end point) and, if so, where I can source it.
