Quick Access toolbar doesn't retain third party additions

If I open a Word 2007 document (untitled) and save a third party add-in to
the Quick Access Toolbar, that item vanishes the next time I open Word.
Here's how I produced the problem;

Word 2007
Windows XP and on Windows Vista

Install third party add-in
Launch Word 2007.
Add a command to the QAT by right-clicking on the command choosing "Add to
Quick Access Toolbar"
Click File > Save
Save the file as a regular 2007 file (.docx).
Close Word.
Launch Word from the another document saved in the past, not from the Start
Menu shortcut.
Close Word.
Launch Word from the document just created in the steps above.
The command disappears from the QAT.

What I've learned so far:

QAT settings are not actually stored in Normal.dot according to the
following page:
http://support.microsoft.com/kb/926805/en-us
This page states that word keeps QAT settings in:
C:\Documents and Settings\<username>\Local Settings\Application
Data\Microsoft\Office\Word.qat

If I follow the procedure above, when I launch Word from the another
document that I have saved in the past, the Word.qat file simply disappears
once I exit from Word. Without that file present, there is no record of what
customizations have been done.

This is as far as I've gotten. If there is a solution to this, I'd like to
learn about it.
