Radio Box appended to H1

Hi

I need to create a radio box (two fields to be ticked by the user), and have it automatically appended to Headlevel 1, such that each time a headlevel 1 is created, the radio box is appended to it. Is this at all possible?

Create an instance and select it and create an autotext entry from it.
