Re-Order Function??? Please help

I have a serious issue that I can not seem to resolve. I wish I could post
the actual spreadsheet as it is hard to explain....but here is my best attempt

I need to recreate this summary page

My data is as follows: (account names and order history by month...see
below). The total depletions, and average deps are not the problem....that
is easy..but creating the formula for re-orders is the problem......

Hi,
I'm a buyer. I find your data a little confusing. one
thing i notice - no lead times.
The formula i use in my spreadsheet is:
order point = average days use * lead time.
average days use is the average usage over a 6 month
period.
This is for each product we carry.
