recieved telephone call note pad design

hi there folks.
I an looking for a jota template for recieved telephone calls,i,e.-
Name, Time of call, reason for call, company etc.
I am using microsoft office 2003 and have all extras.

Assuming you want a template for OneNote, there are lots and lots of
templates for all Office programs on the Office Online site

A quick search of the tempaltes yielded this OneNote template, which can be
further modified or can provide a starting point for you to create your own
