repeat text input automatically table template

I need to repeat data input into a table template into prescribed cells. For
instance, a table with cells for date, name, phone number, etc inputs that
will appear on multiple pages of the same document such that I don't have to
manually input the repeating data but will appear in the correct cells
automatically after I've entered the information into the first set of cells.
