Reply Address is Differernt than the Original Sender?!

I received an email from another employee who recently changed her name (and
email address). Her old account was terminated and a new one was created
with her new information.

I created a new contact for her in my list and deleted the old one.

Incoming mail shows her old name with the new address in the FROM header.
Hitting reply brings up her old address with her new name?

Does anyone have ideas on how to kill this craziness?
