Report not filling in data

I have a report that I'm using to print a form. The
report is an exact replica of my form. I want the current
form to print on the report. The report was working fine,
but then I added 2 new fields and they are not displaying
on the report. The lable name is printing and the text
box is printing but the data from the form is not filling
in on the report for these two new fields. I've examined
the VBA code and just not finding my error. Any
suggestions? My fields names are SentVia and RequesterID
