Report with buttons to open specific form for each record

Is it possible to have a report that is based on a query (which is based on
numerous other queries) to have a button that opens a specific form for each
record on the report?

I would like for the user to be able to select an appropriate button which
corresponds to a form based on the results showing on the report.
