Responding with a different form.

I've just created a form that we are going to use submit requests for email
and active directory accounts to the information services department. The
form has a seciton to allow the sender to put in his or hers email address. I
want to automaticly send another form back to that address confirming the
receipt of the first form. If someone has an Idea on how to get that done it
would be great.
