Response form error

I'm stuck and beginning to pull my hair out! I have created a response form
in Publsher and am not really HTML savvy....I don't know which option I
should choose when I work in the "form properties" section of my response
form. My website is: http://www.adamptsa.org. I have a parent survey that
we would like to be able to utilize, but when I click on the submit button, I
receive a message that the page can not be displayed. Can someone tell me,
in EXTREME layman's terms, what I need to do in order to publish this so I
can receive responses? I have tried chatting with my host server, but they
say it is a design issue, not a server issue and won't help me. I"M VERY
IGNORANT and DO NOT UNDERSTAND HTML..... Think of me as a 1st grader in the
school of web design! Thank you.

I get a message that the web site address - http://www.adamptsa.org
doesn't exist.
