Had it working beautifully in fact.
While in Design Mode, I was cleaning up the appearance of
my form when I inadvertently deleted "OptionLabel5".
As a result,the form no longer functions. The underlying
Macro is looking for the missing label. I tried copying
another label but it wouldn't let me renumber it to 5.
Removing the associated Option5 button doesn't appear to
solve the problem (but I did not permanently delete that
for fear of making the situation even worse).
