Rolodex form

You mean on PAPER??? Why? I thought the whole purpose of programs like
Outlook was to eliminate paper. Note that an Outlook contact file can be
taken on the road in a PDA; this would be much more unwieldy with a deck of
rolodex cards.

You can create custom paper sizes and create a custom view with the fields
you need printed. You can also do a mail merge to Word to fine tune the
look of the cards. (Word also supports custom paper sizes and if you are
using rolodex stock designed for printers, word might have a template for
the paper size.)
