Room booking form

Does anyone have or know where i can find a form that will allow people to
book a room in a large facility for workshops, meetings, etc. and be able to
also note what equipment and set up they will need during the time of thier
booking?

Atwork we use Outlook for scheduling conference rooms. You need to know
whether a room is available before anything else. I would suggest a simple
table for the rest of the applicable data.
