Rules not processing automatically for messages received from outs

Outlook 2007 setup to use an account that runs on exchange 2003
Created a rule so that any message that has a subject that contains the word
"SPAM" moved to the junk email folder.
This rule does not process automatically for email messages that are
received from outside the exchange server.
so if the user sends himself a message with the word "SPAM" in the subject
line, it is moved to the junk email folder.
if I send him the message from a personal email account, not hosted on the
exchange server, the rule does not work.
if a message from any other email account, which is not hosted on the
exchange server, sends him an email with the word "SPAM" in the subject line,
it does not move the message.
If he manually goes into "Rules and Alerts" and runs the rule manually, it
will move all the previously unmoved messages (regardless of where it is
from) that have the word "SPAM" in the subject line.
