Run-time Error 4248 and Customized Toolbars

Run-time Error 4248 - This command is not available because no
document is open. is the error some users have been experiencing when
attempting to close a document that contains a customized toolbar.

Each document is different as is the toolbar. When I delete the
toolbar from the document by using the Organizer, the document is able
to close. However, if I recreate a new customized toolbar, I receive
the same error.

There was an article in the Knowledge Base - but it didn't seem to
relate to what is happening.

Any suggestions are very much appreciated.
