Safety alert

Hi all,

I work in an automotive plant and am looking for a safety alert template
that would allow a user to imput a picture and some description of a pending
safety issue. Not sure all it should entale, but a start of where to look
would be greatly appreciated.

Your request is pretty specific. I think you're going to have to develop a
template on your own.
