I have a user who is requesting a salutation field on the contact form, She
added a user1 field in the phone list view, and started putting salutation's
in there.. now she wants that to show up in the contact form for everyone in
the contacts.

how can I do this.. do I have customize the form ?

any thoughts ?

Yes, you must customize the form to add any field not supplied by Outlook.
What's a "salutation"? In a letter it would be "Dear Sir," or "To Whom it
May Concern:". I can't imagine why you'd want than in a contact form.

Anyway, see this: http://www.outlookcode.com/article.aspx?ID=35

Sometimes it can also be doctor, professor, Mister, Miss, or even if say
they go by there middle name, but you need to keep it organized with their
full name.

I thought I would have to customize, but do you know if I customize it will
work then for all contacts, its not just new ones correct..

click the full name button - there is a selector for title. If you just type
it in (i.e. Dr. Mark Smith) outlook will recognize Dr as the title.
