Sample Form for Flight Scheduling & booking

I am about to design a database to schedule & book
all our employee airline flights.
I am just beginning and am wondering if there are
any sample forms I might be able to look at, that will
give me some ideas, & help prevent some pitfalls.

I'm not looking for a completed database or form, just
a sample or two.

Any ideas.....?
