Views are not associated with a form, but with the folder in which items are
saved or with your mailbox or .pst file as a whole. It's not obvious how,
but you can use a PST file to back up your views. Here's a link to an

In any folder to which you might apply the view, you would also need to add
the custom fields that you want to see in that folder.
