saving a mailitem (custom form) into database with attachments

I am using Outlook 2000

I want to create a action menu item [like published form] for forwarding
selected/highlighted [inbox] mail item using Custom form [which gets some more
inputs] and then

i want to forward this mail and other information to another email id which is not
visible in the screen [custom form] and also want to store the information to Database like SQL Server using Outlook VBA

Any direction would be helpful
