Saving and emailing messages with form fields

How are you creating the messages? HTML form fields or Outlook custom form fields?

I open a new mail message. On the view menu I select forms. On the forms
toolbar I select drop down form fields, and configure them accordingly. I
save the email message as a template.

Ah, that's the Word toolbar for creating forms in documents. You can't gather data with email messages using such forms. Consider an Outlook form instead -- see http://www.outlookcode.com/d/forms.htm and http://www.outlookcode.com/d/messageform.htm
