saving and sending custom forms

I created a cuatom form and am trying to send it to a public mailbox. This
form is connected to a table and once it is sent to a mailbox, an
administrator is supposed to export the data in this form to a table. The
problem is that when it is sent it keeps sending as a regular message, not an
actual form. When I open it up, it looks the same but has lost its
connection to the table. Is there something I am missing that is preventing
the form from being sent as a form, not a message?

I think it depends on how you did publish the form. If you published it and
said "Yes" when asked if you want to save fileds with form definition, then
the form template would be included when teh form is sent. If you said "No"
then the form template would not be included and you would see it as a
regular message.

Well It seemed that whether or not I checked that box, it still would lose
its functionality once it is sent. Still not sure what is happening
