Somebody mentioned that one could scan a form, set it as the background and
add form fields on an upper layer that, when printed, would look like a
completed form? How would one go about this?

You just described how one would go about this.

Scan a form.

Set that form (which will be a big picture) as the background for your
template by putting it in the header of the template.

In the document body insert a table (without borders) or frames (without
borders) and formfields for you to type in. What you are talking about -
looking for - is what Word calls an "online form." Check this in help. For
more about online forms, follow the links at <URL:

Chapman's series of articles.

Note, this is a _lot_ of work for a one-of project. To just fill in a form
on your computer, check out the software that came with your scanner.

Ah... this is the step I was missing, to put it in the header of the
template.
In the document body insert a table (without borders) or frames (without
borders) and formfields for you to type in.

The table is what will keep the fields in the same spot instead of moving
around as I fill them in, correct?
Note, this is a _lot_ of work for a one-of project. To just fill in a form
on your computer, check out the software that came with your scanner.

I'll be using this form a lot, and they won't let me completely computerize
it, so I'll just type over it.
