It will be easiest if you paste the scan into the header of the blank document,
set its text wrapping to Behind Text, and position it properly with respect to
the edges of the paper. (See
http://sbarnhill.mvps.org/WordFAQs/AnchorToHeader.htm.) Print that and hold the
print up to the light along with the original document, adjust the size and
position if needed and print/test again, etc. until it's right. This can get
very finicky.
Once it's lined up correctly, you can add the text boxes so they match up to the
scan without affecting the scan's position.
When you're ready to print a filled-in document, delete the scan from the
header, print, then hit Undo to get the scan back. If you have lots of these to
do, you'll want a template containing a macro to handle these operations
automatically.

Yes, but be aware that when you try to print with blank certificate stock,
it may or may not line up quite as nicely.
