Scheduled meetings disappearing from users calendar

Our MD's PA replies to meeting requests for him "on
behalf of". Accepted requests then populate his calendar.
Some of these meetings are subsequently "disappearing".
When she checks the reply in her Sent Items it shows the
message "This meeting is not in the Calendar; it may have
been moved or deleted. *** has accepted on behalf of ***".

Doesn't happen every time. Any ideas???
